If one vessel gives a single blast to signal port-to-port, does the other vessel respond? Also, how would one communicate this intention on VHF, particularly if you don't know the name of the other vessel?

You would signal back the same meaning you are in agreement. If you don't know the name of the vessel you would say "Vessel approaching ... " Any identifying remarks like position, heading, size, etc so that they know you are talking to them. If you have an AIS you will be able to see the name of the vessel - so long as it is broadcasting AIS which all commercial vessels world wide are requires. Many recreational vessels also do.
